What To Bring: Please bring copies of transcripts and test scores, as well as a completed Medical Waiver.
Medical Waiver: Please bring completed medical form with you, signed by a parent if camper is under the age of 19. Download Form Here.
Insurance: Faulkner University requires each camper to have proof of medical insurance. Camp participants are required to bring a front and back copy of their insurance card at time of check-in.
